Asbestos Operative Paul Hides was carrying out boiler inspections when he came across a hatch that had been left open and unattended. The hatch in question accessed the sub-basement area on unit two.
Paul rang his supervisor, Tom Watling, and asked if he could visit the location as he did not want to leave the area unattended to prevent a potential trip or fall incident. Paul was unsure if the hatch was open for a specific reason and as such whether it would need to remain open. Both parties could not identify a working party so contacted the Central Control Room, who confirmed no t-card was currently in force for the subbasement and informed them they were closing the hatch.
The incident was reported to the Risk Team and a SEAR Incident was raised. Paul’s awareness ensured a significant hazard was eliminated preventing a potential injury due to a fall from height. The incident was highlighted and used as a learning tool in following meetings.
